Main Purpose
Young London Today provides support and Semi-Independent provisions for young people leaving local authority care, to transition into adulthood and to independent living, education and/or employment.
We are seeking to recruit an experienced, confident, and dynamic Care Manager with a minimum of 2 years proven management and supervisory experience working with Care Leavers aged 16-21. Must have a NVQ Level 4 qualification (or higher) in Health & Social Care. Knowledge or experience of working within the Ofsted framework would be advantageous.
We are looking for someone that loves the work they do and that is passionate about helping young people. The post holder must be able to lead and support a team and carry out staff supervisions. Knowledge of creating staffing rota’s is desirable. Must have good IT and literacy skills and be able to write and proof-read reports. Must be organised with excellent time management skills and be able to work under pressure.

Key Accountabilities:
- To manage and support an allocated caseload of young people aged 16+ by having key responsibility to ensure the provision and co-ordination of practical, personal, emotional, social and financial advice to enable a successful transition to adulthood.
- Assess the needs of young people by actively contributing to the assessment and preparation of the Pathway Plan to enable young people to achieve independent living.
- To oversee an allocated caseload of services and staff ensuring safe and high-quality service delivery in line with Ofsted National Minimum standards and Children’s Homes Regulations.
- To ensure that services are prepared and ready for inspections both always announced an unannounced. Ensuring that YLT’s organizational KPI’s are met monthly.
- Contribute to the involvement and participation of young people in the development and delivery of their Pathway Plans and develop creative approaches that will fully engage young people in the process i.e., seeking service user written feedback monthly and effectively implementing agreed changes and recommendations. Ensuring that young people are fully involved in and consulted in their care.
- Ensure that timescales to review Pathway and Care Plans are achieved and be responsible for convening additional reviews when circumstances significantly change.
- To assist the management team with effective Rota planning and suitable matching of keyworkers to young people and placements.
- To assist the organization with any Safeguarding or Disciplinary matters that may arise in the course of your duties. To promptly report any concerns to the appropriate Senior Manager
- To attend identified training and supervision and ensure that staff supervision and appraisals are effective and delivered within timescales.
- Co-ordinate how services are provided for the young person in a timely way, developing constructive professional relationships to enable the young person to succeed in their transition to independence and adulthood.
- Proactively manage professional relationships with social services and other external organisations.
- To offer consultation and support to social workers and other who are actively involved with young people, developing effective solutions to meet their needs as they move toward independence.
- Develop and explore new ways of working in partnership and collaboration with other agencies and professionals to enhance the lives of young people.
- Be responsible for your own development plan, and to promote self-development within staff.
- Report any concerns regarding poor performance or conduct in a timely manner. • Manage the duty phone on a rota basis and respond to emergency calls etc. out of hours when required which may include personally covering shifts.
- Engage and motive the young person as far as practicable, motivating them to accept and use resources effectively.
- Undertake and advocacy role on behalf of young people and support them to attend interviews etc. relating to their progress into employment, education and training.
- Accurately assess, evaluate, and report on key outcomes for young people
- Develop and awareness of financial management issues including welfare rights to assist young people to develop individual financial capability.
- Undertake one-to-one and group work with young people aimed at practical support and preparation for leaving care.
- Write reports in relation to individuals as required. Maintain accurate case records on young people in line with departmental standards and guidance. Reviewed as required so that young people are supported to achieve independent living.
- Ensure that the diverse needs of young people are met taking into account their ethnic heritage, religion, language, disability and that gay and lesbian young people received a fair service based on need.
- Fully contribute to and use supervision to effectively raise issues of concerns and as a tool for personal development.
- Undertake relevant training as necessary to enhance personal skills and professional development.
- To deliver training and induction when requested.
- To undertake any other work appropriate to the levels and general nature of the duties of the post.
- To carry out all duties with regards to Health and Safety Regulations, GDPR and Data Protection legislation.
- Required to form part of the On-Call Duty Team
- Undertake other duties pertaining to the post as requested by senior management.
