TBC Healthcare is an award-winning provider of Tier 3 multi-disciplinary specialist weight management. The organization has been providing the NHS with adult services in London and the South of England since 2009. We set out to empower, motivate and inspire people to manage their weight and make lifestyle changes, which enable them to reach their full potential. We offer a fully comprehensive clinically-led service, incorporating dietetic, activity and behavior change support. Our patients are at the heart of the service, offering flexible treatment to suit all needs. We are looking for a Patient Coordinator to join our Care team.
Main duties of the job
The patient coordinator is a critical role managing the patient’s experience of the service from referral through to discharge. This includes making initial contact with individuals referred to the service and organizing their treatment pathway by making appointments and supporting them to ensure their satisfaction with the service. The successful candidate will be a self-starter able to manage their own workload. Communication is at the heart of the role and requires an ability to listen with empathy as well as being able to liaise with other professionals and stakeholders.
General Duties
- Entering patient data into the practice management system
- Carry out initial telephone interviews with patients being referred to the service
- Book appointments into the system
- Complete and send correspondence
- Complete all requirements relating to data collection for their own caseload.
- Keep coherent records of all clinical activity in line with service policies and procedures.
- Communicate, in a skilled and sensitive manner with patients, family/carers and others as appropriate.
- Work closely with the Lead Service Manager on all aspects of patient care.
- To build working relationships with the rest of the care team and to communicate with the different clinicians.
- To work with clinical governance on alerting and resolving issues that may arise relating to any complaints or issues of safety that may arise in a timely manner.
- Maintaining confidentiality of information in line with the requirements of the Data Protection Act and ensure that client confidentiality is protected at all times.
Desirable
· Have experience of working remotely in a team
· Have people facing experience and confidence to manage expectations
· Have knowledge of a medical environment and confidentiality protocols
· Experience related to weight management
· Excellent communication skills
· Be self-motivated to manage a caseload remotely
· Understanding of hard to reach populations
· Ability to listen with empathy and be non-judgmental when dealing with complex issues related to patients living with obesity.
Education & Qualifications
- Educated to GCSE standard
- Good written and communication skills in English language
