We are seeking enthusiastic, committed and experienced Social Workers to join our Learning Disability Service within Warwickshire. The team works across the whole of Warwickshire but has bases in Warwick and Bedworth. The team consists of Social Care Practitioners, Social Workers, Team Leaders, Senior Occupational Therapists, Occupational Therapists and an Occupational Therapy Assistant.
There is a vacancy for a Level 3 Social Worker (30 hours per week)
There are two vacancies for Level 2 Social Workers (37 hours per week).
Key Requirements:
As a member of the Learning Disability Team, you will be flexible, innovative and have an up-to-date knowledge and experience of legislation and policies in the social care arena. The Team works with Adults with Learning Disabilities and Autism Spectrum Disorders (ASD) with physical disabilities, mental health, carers and their families.
You will manage incoming work and triage referrals, gather additional information in line with participating as the office cover worker as per duty Rota. You will be allocated work in line with the team’s priorities and hold a case load in adherence to Statutory Responsibilities and Care Management functions. You will carry out work relating to adults with Learning Disability and Autism which may include Adult Safeguarding or following Adult Self-Neglect pathways.
Dependent upon level you may have complex cases and undertake work within the Transforming Care Programme or work with customers subject to S117 or Continuing Healthcare. You will work collaboratively with our customers, families, and stakeholders.
You will actively participate in peer group and adopt a strength based approach in supporting our customers to remain as independent as possible. You will encourage a community assets-based approach along with reablement, equipment and assistive technology.
As a strong team player, you will be required to contribute to the development and promotion of the Learning Disability Service.
